The Barrie and Community Family Health Team (BCFHT), one of Ontario's 200 Family Health Teams, reports that they have experienced a data breach earlier this year, potentially compromising sensitive patient information.
The breach was initially detected in March, and after a thorough investigation by cybersecurity experts, the BCFHT is in the process of notifying all affected individuals.
According to an email sent to some patients, personal details may have been compromised including:
- names,
- addresses,
- health card information,
- medical notes
- For a limited number of individuals, even more sensitive data, including Social Security numbers, financial information, and other identifiers
The nature of the attack and the number of affected individuals is not disclosed.
The health team is advising those impacted to be vigilant against potential identity theft or fraud and to avoid clicking on suspicious links or downloading attachments from unknown sources.
