Fundamental Administrative Services reports data breach exposing data of 56K patients

Fundamental Administrative Services, LLC (FAS), a Maryland-based service provider to long-term care facilities, reports a data breach that compromised sensitive patient information affecting 56,325 individuals across dozens of healthcare facilities nationwide. 

The breach was discovered on January 20, 2025, when Fundamental became aware of suspicious activity on its computer network. The investigation revealed that the breach began occurred between October 27, 2024, and January 13, 2025. The attackers copied files containing personal data. 

Exposed data includes:

  • Names
  • Social Security numbers
  • Driver's license or state identification numbers
  • Financial account information
  • Dates of birth
  • Medical treatment information
  • Health insurance policy numbers
  • Medicare/Medicaid plan names

The nature of the attack is not disclosed. The company did not explain why the breach went undetected for months.

The breach impacted residents of 75 long-term care facilities across multiple states that rely on Fundamental Administrative Services for administrative support. Some of the affected facilities include Alamo Heights Health and Rehabilitation Center, Berlin Nursing and Rehabilitation Center, Casa Maria Health Care Center, Horizon Specialty Hospital of Las Vegas, Magnolia Manor facilities across multiple states.

Fundamental has established a toll-free assistance line at 1-855-202-3346, available Monday through Friday from 9 a.m. to 9 p.m. Eastern Time for individuals seeking additional information about the incident. There is no mention of free complimentary credit monitoring or identity restoration services being offered to affected patients.
