The investigation completed on December 30, 2025. The exposed data includes:

  • Full names, dates of birth, and Social Security numbers
  • Home addresses, phone numbers, and email addresses
  • Medical record numbers and dates of service
  • Medicare information and insurance carrier details
  • Diagnostic data, treatment plans, and procedure codes
  • Credit card details and checking account information
  • Behavioral health and immunization records

The number of affected individuals is not disclosed.

Laurel Health Centers is sending letters to all affected individuals and are offering free credit monitoring and identity protection. The company also set up a help line at 1-800-405-6108 for patients to check if their data was stolen.

Security experts advise patients to watch their bank statements and credit reports for unexpected charges or activity and should consider freezing their credit with the major bureaus to stop new accounts from being opened in their names.
