Life Care Services, a senior living operator across multiple states is reporting a cyberattack that affected several of its properties in Florida and Alabama.
The incident was caused by unauthorized access to employee email accounts. The suspicious activity was detected on September 16, 2024. Life Care Services launched a forensic investigation that confirmed unauthorized third parties had gained access to several employee email accounts between September 5 and September 11, 2024. The forensic review confirmed that both staff and resident data had been exposed. Exposed data includes:
- Full names
- Home addresses
- Dates of birth
- Social Security numbers
- Driver's license or state identification numbers
- Passport numbers
- USCIS/alien registration numbers
- U.S. military identification numbers
- Account usernames and passwords
- Financial account numbers and access information
- Payment card access information
- Medicaid/Medicare numbers
- Health insurance information
- Health record numbers
- Patient account numbers
- Provider names and locations
- Clinical information
- Medical treatment or procedure information
- Mental or physical diagnosis information
- Prescription information
- Biometric information
The incident affected at least 10,000 individuals:
- Freedom Plaza Senior Living in Sun City Center, Florida - 4,847 affected individuals
- Freedom Square of Seminole, Florida - 3,473 affected individuals,
- Regency Oaks in Clearwater, Florida - 2,008 affected individuals
- Additionally, Galleria Woods in Hoover, Alabama was confirmed to be affected, but the number of impacted individuals at this facility was not disclosed.
Life Care Services began mailing notification letters to affected individuals on July 11, 2025, and is offering complimentary credit monitoring and identity theft protection services to all impacted parties.
