The Heart of Texas Behavioral Health Network, an providing mental health and intellectual disability services reports a physical security breach that potentially compromised sensitive patient information. 

On November 20, 2025, intruders broke into one of the organization's facilities in McLennan County, Texas, and may have accessed or removed paper patient records. The behavioral health network discovered the intrusion on the same day it occurred and notified law enforcement.

The exposed data includes:

  • Patient names
  • Home addresses
  • Dates of birth
  • Social Security numbers
  • Medical record numbers
  • Diagnoses
  • Treatment or procedure information
  • Medicaid or health insurance information

The breach affected approximately 1,309 individuals.

The organization is notifying all potentially affected patients by mail and is advising affected individuals to review their explanation-of-benefits statements, billing statements, and credit reports for any signs of fraudulent activity.
