Western Wayne, a healthcare provider based in Michigan, is reporting a data breach that compromised protected health information of patients.
The attack to Western Wayne's network began on January 28, 2025. The breach was discovered two days later on January 30, 2025. It took until September 30, 2025 to complete the investigation and confirm that protected health information had been stolen. The exposed data includes:
- Names (first and last)
- Addresses
- Phone numbers
- Email addresses
- Dates of birth
- Medical diagnoses or conditions
- Social Security numbers
- Health insurance information
The nature of the attack and the number of affected individuals have not been disclosed.
Western Wayne is providing affected individuals with complimentary single bureau credit monitoring services for 12 months that include credit file change alerts, fraud assistance, and identity protection support. A dedicated call center has been established at 1-800-405-6108 to address inquiries.
