Integrated Orthopedics of Arizona, a medical practice operating in Scottsdale and Phoenix, is reporting a data breach affecting patient information stored in its email systems. 

The practice detected suspicious activity in its email tenant on April 7, 2025. An investigation conducted with the assistance of third-party cybersecurity experts confirmed unauthorized access to the email system, and that some emails had been copied. 

Exposed data includes

  • Names
  • Addresses
  • Dates of birth
  • Medical record numbers
  • Patient ID or account numbers
  • Medicare numbers
  • Medicaid numbers
  • Health insurance ID numbers
  • Health insurance group numbers
  • Medical diagnosis information
  • Medical treatment information including dates and locations
  • Doctor's names
  • Laboratory or test results
  • For a small subset of individuals, driver's license numbers

The breach impacted 2,916 patients. The affected individuals include both current and former patients who had either visited Integrated Orthopedics of Arizona for healthcare services or whose information was provided by other healthcare providers. 

Integrated Orthopedics of Arizona reported the incident to the authorities and sent out patient notifications.

Affected patients have been advised to monitor their financial accounts and credit reports for signs of fraudulent activity, consider placing fraud alerts or credit freezes with major credit bureaus, and remain cautious of unsolicited communications requesting personal information that could be related to the exposed data.
