The incident was caused by an unauthorized access to its IT network between January 16 and February 4. The breach was discovered following the detection of "unusual activity" in the hospital's Information Technology department. Immediate actions were taken to secure and isolate the affected IT systems, and a comprehensive investigation was launched with assistance from an external cybersecurity firm, while law enforcement was notified of the incident.
The investigation revealed that an unauthorized entity had gained access to and possibly acquired files containing sensitive patient data. The compromised information includes:

- dates of birth,
- medical record numbers,
- health insurance plan member IDs,
- claims data,
- diagnoses,
- prescription details,
- Social Security numbers,
- driver’s license numbers.

Trustpoint Rehabilitation Hospital has mailed breach letters to the impacted individuals. For those whose Social Security and/or driver's license numbers were compromised, the hospital is offering complimentary credit monitoring and identity protection services.
