Texas Panhandle Centers has reported a potential data breach impacting its clients. The incident was discovered on October 17, 2023, when an unauthorized entity accessed the organization’s computer systems. Upon detection, the breach was contained, and an investigation was initiated with the aid of cybersecurity experts to determine the extent and nature of the accessed personal information.

By April 19, 2024, the investigation confirmed that a limited number of files were likely accessed by the unauthorized party. These files contained sensitive information, including:

  • Full names
  • Dates of birth
  • Driver’s license and/or state identification numbers
  • Medical record numbers
  • Clinical information (diagnoses/conditions, lab results, medications, healthcare claims, treatment details)
  • For some individuals, Social Security numbers might also have been compromised.

The nature of the attack and the number of affected individuals is not disclosed.

Texas Panhandle Centers has begun notifying potentially affected individuals on May 9, 2024. Those whose Social Security numbers were involved are being offered complimentary credit monitoring services.
