Adapt Integrated Health Care, a healthcare provider based in Roseburg, Oregon, reporteds a data breach involving TriZetto, a third-party vendor that supports OCHIN, Adapt's electronic medical record provider.
Adapt was notified about the hack on December 10, 2025, after OCHIN reported that an attacker broke into one of TriZetto's systems. Adapt claims that the breach did not affect their internal networks.
Exposed data includes:
- Full names
- Social Security numbers
- Dates of birth
- Contact information
- Health-related and insurance information
The number of affected individuals is not disclosed.
TriZetto plans to mail notification letters to affected people starting in February 2026. The vendor hired Kroll to manage the response, which includes identity theft protection and a dedicated call center.
Adapt is now checking its vendor security rules and monitoring OCHIN to prevent future incidents.
Patients should monitor their invoices for unexpected charges and review insurance statements. They should also be very careful of messages and calls requesting data or claiming to be Adapt.
