Allegheny Health Network (AHN) is reporting a data breach affecting patients who received home medical equipment or home infusion services.
The incident occurred through unauthorized access to computer systems belonging to an unnamed third-party IT vendor. The intrusion began on October 11, 2024, and was discovered by AHN on November 19, 2024. Immediate remediation steps were implemented to secure patient information and terminate unauthorized access.
The exposed sensitive patient information includes:
- Names
- Dates of birth
- Addresses
- Social Security numbers
- Financial account numbers
- Health insurance identification numbers
- Other health insurance information
- Diagnoses
- Provider information
- Treatments/procedures
- Dates of service
- Prescription information
- Medical device serial numbers
The name of the third-party IT vendor involved and the number of affected individuals are not disclosed.
The IT vendor, as directed by AHN, is in the process of notifying all impacted individuals and will provide complimentary identity protection services, including credit monitoring. AHN has stated that they have not identified any instances of attempted identity theft or fraud resulting from this breach.
