Healthcare Therapy Services, Inc. ("HTS"), a Greenwood, Indiana-based provider of physical, occupational, and speech therapy services, reports a data breach that exposed sensitive patient information. HTS serves senior living communities, home health agencies, and hospitals.
On April 29, 2025, HTS detected unusual activity involving its email systems and started an investigation with the assistance of third-party cybersecurity professionals. The email system compromise allowed the attacker to access sensitive records stored within the company's systems. The exposed data includes:
- Name
- Social Security number
- Driver's license information
- Financial account information
- Medical information
The number of affected individuals has not been disclosed. HTS claims there is currently no evidence of misuse of the compromised information
The company mailed notification letters to affected individuals beginning November 7, 2025 and is offering impacted individuals 24 months of complimentary credit monitoring and identity theft protection services which includes monitoring, a $1 million insurance reimbursement policy, and fully managed identity theft recovery assistance.
HTS has established a dedicated call center for affected individuals, available Monday through Friday from 9:00 a.m. to 9:00 p.m. Eastern Time at 1-833-274-5072.
