Ochsner LSU Health Regional Urology reports data breach exposing patient information

Ochsner LSU Health is reporting a data security incident that occurred on retired systems that had not been operational since December 31, 2022. The breach, discovered around October 5, 2025, resulted in unauthorized access to patient files containing sensitive personal and medical information from patients who received care at the northwest Louisiana facility prior to the end of 2022.
The investigation confirmed that an unauthorized third party accessed the legacy systems and stole certain patient files. The exposed data includes:
- Names
- Social Security numbers
- Dates of birth
- Medical record numbers
- Information related to care received at Regional Urology prior to December 31, 2022
The number of affected individuals and the nature of the attack have not been disclosed. According to Ochsner LSU Health, the breach was limited to the legacy systems and no other Ochsner LSU Health System or Ochsner Health locations were impacted.
Ochsner LSU Health is mailing notification letters to all patients whose information was involved in the breach. The organization is offering complimentary credit monitoring and identity protection services to individuals whose Social Security numbers were included in the compromised files.
The organization has established a toll-free call center at (844) 572-2718 for patients seeking additional information about the incident.
Update - As of 21st of December 2025, Ochsner LSU Health–Regional Urology reports that the incident affected 4,519 individuals.
