A Nevada-based medical practice, Swift Institute, reports a data breach that compromised personal and health information of patients and individuals associated with the facility. The incident was discovered on January 30, 2025. The investigation confirmed that on or about January 28, 2025, files were copied by an unauthorized individual who gained access to the medical practice's systems.
The compromised information includes:
- Names
- Social Security numbers
- Driver's license or state identification card numbers
- Dates of birth
- Financial account information
- Credit or debit card information
- Medical information
- Health insurance information
- Digital signatures
The number of affected individuals and the nature of the attack are not disclosed.
The organization completed its comprehensive review of the impacted data on December 4, 2025, after which it moved expeditiously to notify affected individuals. The breach was formally reported on December 12, 2025.
Swift Institute is offering affected individuals at least 12 months of complimentary credit monitoring and identity theft restoration services. Affected individuals must enroll within 90 days of receiving their notification letter to activate these protective services. The medical practice established a dedicated assistance line at 833-844-8151, available Monday through Friday from 8 a.m. to 8 p.m. Eastern time, to answer questions and provide support to those impacted.
