UI Community HomeCare, a subsidiary of University of Iowa Health System, reports a data breach in which an unauthorized access had been gained to confidential consumer information.
The breach exposed consumers' names, dates of birth, addresses, phone numbers, and protected health information, affecting 67,897 individuals.
UI Community HomeCare discovered the breach on March 23, 2023, when certain files on their computer system were found to be encrypted, leading to an internal investigation. It was determined that an unauthorized party had accessed the company's computer network and compromised files containing sensitive information.
The breached information varied for each individual but could include names, dates of birth, addresses, phone numbers, medical records, referring physicians, dates of service, health insurance details, billing and claims information, medical history, diagnosis, and treatment information.
UI Community HomeCare initiated the process of notifying the impacted individuals by sending out data breach notification letters.
