The Utah Department of Health and Human Services is notifying 5,800 Medicaid recipients after a coding error caused benefit information letters to be sent to incorrect addresses.
After a system coding error, State Mail and Distribution Services discovered on May 8 that some letters were stuffed in envelopes addressed to incorrect households.
Out of the 5,800 people who may have been affected by the missent letters , in about 200 cases there were Medicaid Health Insurance claim numbers, which for some, may have been a social security number.
Other information, such as the individuals' addresses, first and last names and dates of birth were also included in the letters.
The breach was not believed to be malicious. Utah Department of Health and Human Services is offering credit monitoring services to the 200 individuals whose Medicaid Health Insurance claim numbers may have been exposed.
