Advanced Medical Management LLC, a healthcare management services provider, has disclosed a data breach in which the protected health information of 319,485 individuals was exposed and potentially stolen due to unauthorized access to its network by third-party vendors.
The breach occurred between May 10 and May 13, 2023, and the compromised files contained sensitive data including:
- names,
- addresses,
- email addresses,
- phone numbers,
- dates of birth,
- driver's license numbers,
- Social Security numbers,
- health insurance information.

Notification letters have been sent to affected individuals since June 29, 2023.
