Synergy Healthcare Services, specializing in long-term care providers based in Miami, Florida, reported a data breach that was discovered after the company detected suspicious activity within its computer network in December 2022.
Upon detecting the unusual activity, Synergy Healthcare Services locked down its systems and launching a comprehensive investigation with the assistance of third-party experts. The investigation revealed that on December 15, 2022, an unauthorized party had gained access to parts of the company's computer network.
The breach affected not only Synergy Healthcare Services but also its affiliates, including Consulate Health Care, Raydiant Health Care, and Independence Living Centers.
The breach exposed sensitive information of consumers, including

- Social Security numbers,
- birth dates, signatures,
- insurance details,
- contact information,
- driver's license numbers,
- medical history
- treatment information,
- financial data.
