Terry Reilly Health Services (TRHS), a community health provider based in Idaho, reports a data breach on February 9, 2026, caused by a security incident at a third-party vendor. 

The breach originated at TriZetto Provider Solutions (TPS), a subsidiary of Cognizant that serves as a subcontractor for OCHIN, which provides electronic medical records for TRHS. Forensic investigations by the cybersecurity firm Mandiant revealed that threat actors first gained access to the TPS environment in November 2024 and remained undetected for nearly eleven months before discovery on October 2, 2025. The compromised data includes:

  • Full names and home addresses
  • Dates of birth
  • Social Security numbers
  • Health insurance member numbers and Medicare beneficiary numbers
  • Health insurer names and policy details
  • Provider names
  • Demographic and clinical health information

TRHS is mailing notification letters to impacted patients and has coordinated with law enforcement and external security experts. The organization is offering 12 months of complimentary identity theft protection and credit monitoring services.
