The organization detected the breach on November 28, 2025 and started a forensic investigation with third-party cybersecurity professionals. By March 4, 2026, the investigation confirmed that the threat actor accessed and stole files containing protected health information (PHI) and personally identifiable information (PII).
The compromised data includes:
